These indicators will allow you to determine the type of disorder.12 KEY FACTSMastering quick interpretation of acid-base lab values is a key element to the success of the Nurse. Three components are included in typical lab value assessment: pH, pCO₂, and HCO₃ (Bicarbonate).The first step to acid-base lab value interpretation is to look at pH. Blood pH is normally 7.35-7.45. pH is determined by the amount of hydrogen ions contained in the blood.A pH of less than 7.35 is termed acidosis. Acidosis indicates a buildup of carbonic acid in the blood.A pH higher than 7.45 is termed alkalosis. Alkalosis indicates a buildup of bicarbonate (bases) and/or a general decrease in carbonic acid in the blood.The second step is to examine the partial pressure of carbon dioxide in the blood. pCO₂ is normally 35-45 mmHg and is regulated primarily through respiration.Opposite Respiratory and pH directions indicate a respiratory disorder. If the pCO₂ is not in the opposite direction of the pH, then check the HCO₃ next.Respiratory acidosis is often indicated by a pH of less than 7.35 and a pCO₂ of higher than 45 mmHg.Respiratory Alkalosis is indicated by a pH of more than 7.45 and a pCO₂ of less than 35 mmHg.The normal value of bicarbonate is 22-26 mmol/L. The amount of the base HCO₃, bicarbonate, in the blood is regulated in the kidneys.If the HCO₃ (bicarbonate) is going in the same direction as pH, then the problem is most likely a metabolic problem.The patient with Metabolic acidosis can grossly be determined as Down, Down, Down (Decreased pH, Decreased pCO₂, Decreased HCO₃).Metabolic alkalosis can grossly be determined as UP, UP,

Initially, it is important to treat patients with a severe asthma exacerbation with oxygen. They may also be prescribed corticosteroids, ipratropium or albuterol inhalers. Nebulizer treatments, pursed lip-breathing, and monitoring pulse oximetry are important implementations of treatment. Finally, education begins with teaching patients to carry their inhalers, rinse their mouths after, and avoid triggers.10 KEY FACTSOxygen therapy for acute asthma aims to achieve oxygen saturations of at least 92%. However, the emergency oxygen guidance suggests a range of 94–98% for all situations if possible. Humidified oxygen is preferred.Systemic corticosteroids are often prescribed by healthcare providers for long-term treatment (for anti-inflammatory action to treat reversible airflow obstruction). These medications can be administered via nebulizer, but often come in metered-dose inhalers or in dry-powder inhalers.Anticholinergics are given for relief of acute bronchospasm. Anticholinergics, like Ipratropium, are sometimes combined with Albuterol and given under the trade names Combivent or DuoNeb.Treatment of asthma may be short term or long term. Short term treatments include. Short-acting β2 agonists are administered via inhaler for bronchodilation (albuterol or proventil).Implementation of treatments often begin with medicated dose inhalers for acute exacerbations, or nebulizer treatments. Often after severe attacks, incentive spirometry, is therapeutic to patients.Patient O2 saturation should be monitored with pulse oximetry to make sure that they do not desaturate and become cyanotic. Caregivers should aim to keep patients above 95%.Patients are urged to use pursed-lip breathing exercises during exacerbations, as this improves ventilation, releases trapped air in the lungs, and keeps the airways open longer and decreases the work of breathing. Furthermore, this technique prolongs exhalation to slow the breathing rate and improves breathing patterns by moving old air out of the lungs, allowing for new air to enter the lungs.One educational goal is to help patients understand the importance of having treatment accessible. This means carrying an inhaler in order to immediately treat the exacerbation of symptoms.The patient should be instructed to always rinse the mouth after using an inhaler to prevent an oral candidiasis overgrowth sometimes called thrush.Identifying what triggers lead to asthma is very helpful for patients.
